A server can boot fine, pass a basic stress test, and still be running BIOS or firmware that's several years out of date. Firmware is the layer beneath the operating system — it's what actually talks to the CPU, memory, storage controller and out-of-band management chip — and letting it sit outdated is one of the most overlooked risks in deploying any server, new or refurbished.
Firmware isn't one thing — it's several independent pieces, each versioned and updated separately: the system BIOS/UEFI, the RAID or HBA controller firmware, drive firmware (HDD/SSD/NVMe), network adapter firmware, and the out-of-band management controller (iLO, iDRAC or IPMI — see our iLO vs iDRAC vs IPMI guide). A server can have current BIOS but an old RAID controller firmware, or vice versa — checking one doesn't confirm the others.
A refurbished server that's been sitting in inventory for months can easily be several firmware revisions behind what the manufacturer currently ships — this is normal and not a sign of a badly maintained unit, but it does mean a buyer shouldn't assume "it's tested and working" also means "it's current." Ask specifically whether firmware and BIOS were updated as part of the testing process, not just whether the unit powers on and passes diagnostics.
